Please review the following requirements regarding necessary State Medical forms.
The form requirements differ for each grade level, so please read the details listed below.
Exams must be completed within one year prior to October 1, 2019. Parents must complete and sign the parent sections of each form.
EVERY FAMILY must return a newly completed MEDICAL AND EMERGENCY NOTIFICATION INFORMATION AND AUTHORIZATION FOR MEDICAL TREATMENT FORM - One form per family with specific information for each student in the family on that one form.
If you are a RETURNING POPE FRANCIS FAMILY and have a child going into PRE-K, KINDERGARTEN, 2ND OR 6TH GRADE, you will need your healthcare provider to complete the following forms:

If you are a returning Pope Francis family and do NOT have a child going into Pre-K, Kinder, 2nd or 6th grades, you will not need any new MEDICAL forms.
If you are a NEW FAMILY with children in any grade level:
If your child has an ALLERGY, ASTHMA, DIABETES, OR ANY OTHER MEDICAL CONDITION REQUIRING ONGOING MEDICATION OR MEDICAL TREATMENT, in addition to indicating the condition/s on their Medical and Emergency Notification and Authorization for Medical Treatment Form, please complete and return the Medication Authorization Form and Physician’s Order form. Also, read and retain a copy of our School Medication Procedures.

TRANSFER AND ACQUISITION OF TRANSCRIPTS
For any students transferring in from any other school, please complete this Pope Francis Global Academy Request for Records Form and submit it to your current school. They will send copies of the complete applicable transcripts directly to us. You may, at the same time, request a copy from the transcripts for yourself to keep on file. A new student will not be allowed to start the school year until transcripts have been received.
